Print out our downloadable order form and select what you would like created, then mail your order form, along with your garment to: Jodie Creations, 12441 S Balsam Rd, Dafter, MI 49724. Once we receive your package, we will contact you, either by email or phone, to discuss your options.
Please note that a 50% deposit will be required before we start your order.
